Travel to New York from Ukraine?

I have a friend in Ukraine whom I want to bring for a visit to New York next summer for around a month. What do I have to do here for that to happen and what does she have to do there? Also she'll be 16 next year. Does she still need supervision from her parents or can she just fly with a stewardess or by herself?
